C/O. SHORTNESS OF BREATH SINCE 1 MONTH 

        CHEST PAIN SINCE 1 MONTH 

 

 

HISTORY OF PRESENTING ILLENESS : 

PATIENT WAS APPRENTLY ASSYMPTOMATIC 1NMONTH BACK THEN SHE DEVELOPED 

SOB WHICH IS  GRADE II NHYA ,INSIDIOUS IN ONSET  

CHEST PAIN  INSIDIOUS IN ONSET INTERMITTENT AND GRADUALLY PROGRESSIVE 

 

PAST ILNESSS : N/K/C/O DM, HTN ,THYROID  

 

  THERE IS A HISTORY OF CONSANGUINOUS MARRIAGE
